TERMS AND CONDITIONS

I certify and acknowledge:

That I, fully understand that any packages or block-bookings are non-refundable, and that once said sessions/packages are booked, they may only be used by me or may be transferred to another party, once agreed upon by both myself and Ivan De Freitas. 

The expiration policy of 1-2-1 Personal Training Packages requires completion within a 24 week time period from date of contract start. Personal Training Sessions are void after this time period.

That I, may request the pausing of my Personal Training package due to injury or other inability to complete the session in the time allotted. The request is then at the discretion of Ivan De Freitas and he will revert with an answer within five business days. 

Virtual and/or in-person sessions must be paid for in addition to services above and before their commencement and are non- refundable, once that said sessions are booked they may only be used by me or may be transferred to another party, once agreed upon by both myself and Ivan De Freitas.

That I, fully understand that all sessions types cancelled or rescheduled less than 24 hours in advance will be charged in full. The cancellation policy will be adhered to in all cases and sessions must be cancelled in writing via email or text message other than in the case of a medical emergency (e.g.Contagious Disease or infection, any accident or incident that renders yourself or your immediate family in hospital, loss of immediate family member or advised by a medical professional) in some cases a rescheduling charge of £25 may apply if a studio rental cost has occurred and the rest of the session price refunded or credited back to you.

That I, fully understand that session times may not be extended due to lateness; clients who fail to turn up within 30 minutes of a deemed start time unless notified will be deemed to have cancelled late and charged in full.

That I, fully understand that transformation before and after photos may be used for marketing purposes unless advised other wise due to religious, personal, or contractual reasons.

That I, have read this Liability Waiver, Terms, and Conditions form, understand and agree with each of the foregoing points, and have received a copy of this release form on this date. A printed name and e-signature will be taken as agreement of this liability waiver form and also confirms that the information I have provided to Ivan De Freitas and DeFitness LTD is correct and true.
